Important Forms for GFC Competitive/Rep Teams

 

The following are links to forms (PDFs) that you will need in a typical season:

OSA Competition & Travel Management System (CTMS) List of all OSA sanctioned Festivals/Tournaments; access to online forms including AHEG, ATF, FAF, TAF, ITC. Requires you to create a “login” to access AHEG (application to Host an Exhibition Game) & ATF (Application to Travel Form). 
GFC Revenue Form To be included with all deposits made to Team Account Print 2 copies for each submission. One will be returned to you as a “receipt” for deposit after processing.
GFC Expense Claim Form To be submitted with all expense claims to be made to Team Account Print 2 copies for each submission. attach ORIGIAL receipts for all items included on form; 1 form will be returned to you as a “receipt” for expenses after processing.
GFC Sponsorship Form To be filled out for any team sponsor secured. Ensures proper financial application, documents the usage of the funds, ensures timeliness of sponsor receipts.
GFC Try-Out Form & Waiver To be completed by the parent or legal guardian for any player trying-out for a GFC team that is not already registered to the Club. Keep the completed form for all trial players in your possession until such time they officially join the team; if they do not join the team it is recommended to keep the form on-hand for one full season following their try-out.
GFC Player Medical Information Form To be completed for all players at the start of each season. Keep the completed forms on- as long as the player remains with the team. Due to the privacy nature of information, it is recommended the forms are kept in a sealed envelope in the Coach or Manager’s bag and available at all training and game events. Team officials should be aware of all potential medical issues.
SCAT3 Concussion Baseline Tool & Form Each player should have a completed SCAT3 form. Keep the completed forms on-hand as long as the player remains with the team. Due to the privacy nature of information, it is recommended the forms are kept in a sealed envelope in the Coach or Manager’s bag and available at all training and game events. Team officials should be aware of all previous concussion issues.  Players requiring testing should be tested by Club Athletic Therapists to ensure accuracy of information collected.
Team Playing Out Application To be completed by teams wanting to travel to tournaments or exhibition games outside of the YRSA district. Please allow a minimum of 2 weeks for processing.
YRSA Travel Insurance Acknowledgement Permit To be completed for the team, including all players that are travelling to a tournament or game outside of Canada Players must be covered under personal travel insurance for specified trip dates.
OSA Player De-Registration Form To be completed for a registered Player that wishes to leave the Club and play with another Club during a season. A player that is moving from one Club to another cannot do so without the completion and approval of this form through the YRSA Registrar.
OSA Player Transfer Form To be completed for a registered player that wishes to transfer from one Club or team to another Club or team within the same district in a season. Bothe the releasing and receiving club are required to complete portions of the form and it requires District Registrar approval.
OSA Temporary Eligibility Permit (TEP) To be completed for a registered player that would like to play in a league game or exhibition game for a different team/Club temporarily. This form allows the player to be a guest player for a tournament or exhibition game; it also provides the fleixibility for the player to play in a higher age group, higher league level, etc. temporarily.
OSA Trial Registration Permit (TRP) To be completed for players not currently registered with any team in Ontario for the current season that wished to trial with your team during a season. The form allows for a 15-day trial period that can include a League, Tournament or Exhibition game. It requires District Association validation.
